Insurance is a contract between you and your insurance plan. If you have any questions or concerns about your contract for coverage, please contact your insurance plan directly for clarification. Please be aware that some services that we provide may not be covered by your insurance contract. We will bill insurance companies as a service to our patients. In order to properly bill your insurance company, please present your insurance card and inform us of all relevant insurance information including primary and secondary insurance. Please also keep us informed of any changes in your insurance information. We try to estimate what your insurance company may pay; however, your insurance company makes the final determination of your eligibility and benefits.
Your contract agreement with your insurance plan may require payment of copays, coinsurance or deductibles. Please understand that we must collect these copayments, coinsurance and/or deductibles or it could be considered a violation of billing compliance and/or an act of fraud by your insurance plan. The required payments are due at the time of your appointment. We accept cash, checks, all major credit cards including but not limited to MasterCard, Visa, American Express, Discover, as well as debit cards and gift cards.
Our credit card processor (STRIPE) has been evaluated by a payment card industry (PCI) certified auditor and received PCI Service Provider Level 1 Certification. This is the most stringent level of certification in the payment industry.
After the visit, we file a claim with your insurance company as a courtesy to you. Your insurance company then makes the final determination of your eligibility and benefits. If there is a change from the original patient responsibility estimate, we will either refund you or bill you the difference. If you have questions or wish to dispute your insurance company's final determination of benefits, then you should contact them directly via the phone number on your insurance card. As a health care provider, we are considered a "covered entity" and therefore we must follow one of the strictest data security policies as outlined by the Federal laws called the Health Insurance Portablility and Accountability Act (HIPAA). This group of laws has 3 main parts, the Privacy Rule, the Security Rule and the Breach Notification Rule. The Privacy Rule requires us to safeguard your protected health information (PHI) including information about payments. The Security rule includes security requirements to protect electronic PHI.
